I know the paperwork is state mandated but some of the wording should be changed to fit specific circumstances. Instead of saying I'm responsible for the payment, maybe say if I'm the party responsible for payment....
Just a few sections were written for **** scenario and not allowing you to decline if it didn't pertain to your situation.
